Firefox Homepage

I start Firefox most days on boot-up but frequently have to make two attempts. The first one generally opens one tab with a blank page. When I click the icon again to open a second window my homepage starts as normal. It's not a big problem but annoying. Can anyone explain the cause please?

Also, when I do get it to start there is always an extra tab, the 'Firefox Add-ons' tab. I checked the tabs in Options but don't see a solution there so obviously missing something. How can I stop this tab appearing at each startup?
