NTOSKRNL-HOOK virus on Vista - Need help removing

OK....I've had some help from Microsoft....with little success (more success from google searches) on finding help to identify and remove virus file listed by McAfee as "NTOSKRNL-HOOK" and "GenericRootkit.d!rootkit". If I disable all the startup functions and come into safe mode, I can do a manual McAfee scan, which finds the files and supposedly removes them. If I do the same scan again, they're found again and "removed" again. Obviously, these aren't being removed.

I'm running the McAfee scans in Normal-mode now (with only McAfee functions enabled), and it appears to have detected an item, but did not fix it. What advice is there to get rid of this virus?
