Need help removing twext infection

I'm usually pretty good at this, but trojan/rootkit has definitely outsmarted me. I've tried everything I could think of to remove it including manual deletion in safe mode, but I have not been successful. It attacks winlogon and reloads itself. I've tried going in and fixing the userinit in the registry, but it keeps reverting it back. AVG does not detect the virus, but McAffee lists its components as:

(Since McAfee gives 2 different names, I might have 2 viruses, but since there's no exe file for FakeAlert, I assume they are either components of twext or remnants of an older virus I successfully removed.)

I have 2 computers (one is not infected), so I have reliable internet access on one of them. The infected computer is my Dell Inspiron 2200 laptop. It has about half a gig of memory, and it's 35 Meg hard drive has about 25% free space left on it.

Thanks for providing this service. I run a business from home, and I have no IT person on staff or on call (although I'm usually able to fix most problems I encounter myself). I missed a whole day of work yesterday trying to repair my laptop, and I was getting very frustrated until I came upon this website.

UPDATE

I believe I was finally able to remove twext, and ntos (as well as everything McAffee referred to as Spy-Agent.bw!mem). However, I have not been able to remove the Generic FakeAlert.k virus, which now manifests itself as UACF3B1.TMP. It looks like this "UAC. . ." trojan operates via driver files (.sys), so I went through my non-plug-and-play drivers and uninstalled/disabled drivers such as catchme.sys, beep.sys, and avimon.sys. Everything else in the list seems like it belongs there (although I'm far from an expert on this topic).

There are now no suspicious processes automatically appearing in my task manager (other than those related to MyWebSearch, which is considered by some to be spyware, although my wife uses MyFunCards, of which it is a component, and we've never had a problem with it since it was installed over a year ago).

Now that there appears to be only a single infection left (although I could be wrong), I can tell you which of my symptoms are attributable to this UAC. . . pest. It creates an error when running certain exe files, most notably McAffee virus scan (although if you don't close the error box, the scan continues to run, and you can even view the details when it's done). It also hijacked my browser (Firefox) and redirects me away from any site having to do with cleaning up my computer, most notably lavasoft.com (I was trying to update my Ad Aware definitions which are 600+ days old) and Mcaffee.com. It either gives an "address not found" page if you type the address directly, or it redirects me to obnoxious websites like couponmountain.com, windowsclick.com, san.xmlsearch.miva.com, icityfind.com and blinkx.com, if I attempt to get onto the legit site via a hyperlink.

My browser is also directed to websites consisting of IP addresses, such as 76.9.16.153, 64.111.196.117, 206.161.121.82 and 216.195.52.100. Particularly troubling are redirects to http://206.161.121.82/search.php, which directs me to help identify click fraud and asks me to click on the search phrase I typed, while it transfers data to my computer.

As I have already killed all unknown plug-and-play drivers, and there are no more malicious exe files I can identify, I have not yet figured out how this program can be stopped.

Since the status of my system has changed since I posted my logs, I am reposting new ones.

SECOND UPDATE

I think I'm clean now. I was able to kill the UAC virus with Malwarebytes. Originally, it wouldn't install. I renamed it, and it did fine. Then it wouldn't run, and renaming the exe file did not do the trick. What ended up working was clicking on the executable uninstall program that came with it, then clicking on cancel, which left the program open for me to do the scan and repairs. The program also found a few remnants from the rootkit I had removed (and also of a virus I removed 2 years ago). I then uninstalled AVG (since I already run McAffee) so I don't have interfering AV services (I needed AVG because McAfee wouldn't do a safe-mode scan).

Since I'm not an expert in this stuff, however, if any of the admins here have some time (between fixing some of the higher priority problems people are having), can somebody advise me how to be at least 99% sure that my system is actually clean? Also, I could use a little advice on tightening up my system to prevent this from happening again. I use a secure wireless network between my 2 computers, my printer/scanner and my son's Wii. I use the internet on my desktop (which was not infected) mostly for business, although I frequent wikipedia, flickr and facebook (which I believe are pretty safe). The laptop is mostly a play computer these days. My wife uses it to read the news and send e-cards. I use it mostly for photography and flickr. My son uses it for homework, although I occasionally find out he installs a game or two (which I've discussed with him thoroughly after missing work for the last 3 days to fix the computer). I have no idea how I got these viruses, and I expect there may be a serious security issue.

I already had McAffee running on the laptop when this happened, but today I reinstalled Ad Aware, which was about 2 years out of date on the laptop and never installed on the desktop.

Any other suggestions?

We need to see some information about what is happening in your machine. Please perform the following scan:
  • Download DDS by sUBs from one of the following links. Save it to your desktop.
  • Double click on the DDS icon, allow it to run.
  • A small box will open, with an explaination about the tool. No input is needed, the scan is running.
  • Notepad will open with the results.
  • Follow the instructions that pop up for posting the results.
  • Close the program window, and delete the program from your desktop.
Please note: You may have to disable any script protection running if the scan fails to run. After downloading the tool, disconnect from the internet and disable all antivirus protection. Run the scan, enable your A/V and reconnect to the internet.
