o2sdx64.sys is related to a Media Card reader. I'd suggest disabling that device in Device Manager to see if that fixes it. If so, then download a new copy of the driver, uninstall the old software and then install the new one.
My browser caused a flood of traffic, sio my IP address was banned. Hope to fix it soon. Will get back to posting as soon as Im able.

I always hope that the answer is just that easy - but sometimes the memory dumps don't cooperate. in those cases it takes several memory dumps, and maybe using another tool to get an idea of what's causing the issue.

If the above "fix" doesn't work, post back with the next dump file report and we'll work on it some more.
