Lsas.Blaster.Keyloger problem

I too have encountered the Lsas.Blaster.Keyloger problem and attempted both remedies that were posted. Logs are attached below. The "Malwarebytes Anti-Malware" said it was removed but, both times I restarted the computer and Icons and pop ups are still there. Any suggestions? Should I reset the computer to factory settings? Thanks.

Try mbam one more time

Please reboot the computer
Open MBAM and click Update tab, select Check for Updates,when done
click Scanner tab,select FULL scan
After scan click Remove Selected, Post new scan log for review
