The registry cleaner is a dangerous tool (I broke an internet connection today doing that!
If you absolutely insist upon it - the best thing is to:
1) Backup the registry
2) Learn how to restore it if Windows won't boot for you.
The biggest issue is in knowing what can be safely removed and what can't be safely removed. CCleaner selects all of the issues automatically, so if you take it's word you'll delete everything that it finds. What if it's wrong?
