sometimes the ping rate it 500 and sometimes it's 90.

It's like the freeway - it's great when no one's on it, but once the traffic builds up you'll slow down a lot.
Also, each different connection that you get is affected by the path it takes - and each path is not necessarily equal to any other path. Try running a tracert to your game server - look at all the connections that it makes to get there. Any one of them can have problems, and even without problems, traffic on any one can slow you down immensely.
