I managed to delete some trojans after running a live linux boot disc, plus GMER which suggested that a Rootkit was found, but the log for that was corrupted.
I placed the infected drive in another pc and ran Malwarebytes which detected a further 30 odd files, and also tried Kaspersky's TDSSKiller which reported two suspicious files, one to do with a Fraps installer, the other I can't remember (Doh!). I also ran the Microsoft Safety Scanner and ESET online virus scanner both of which scanned over 500000 files and reported nothing suspicious
I then placed the drive back in to the pc, it boots fine but still won't run or install any programs. I have tried renaming files and changing file extensions. There are no error messages, when I double click a shortcut nothing at all happens, tried in safe mode and as administrator.
I have also attempted to create a new user, but again there is no response when clicking create a new user.
I can't get Hijackthis to run either.
Any ideas as to what I could try next, before I do a format and long winded re-install?

PS the owner of this pc is a Minecraft and COD freak and is forever downloading extra maps and extensions and doesn't seem to worry about where they come from..
