It (almost) always occurs while playing a game. (World of Warcraft, Civilization V, etc..)
It rarely occurs during other usage.
It always happens twice in a row:
Initial crash during gameplay with following information on blue screen: "0x0000003B (0x00000000c0000005, 0xFFFFF96000084801, 0xFFFFFA6008BACFE0, 0x0000000000000000) [line break] win32k.sys - Address FFFFF96000084801.base at FFFFF96000020000, DateStamp 4de794fc"
Upon restart I always receive a second blue screen that displays: "c000021a [line break] 0x00000000 (0xc0000001 0x001004f0)"
****Note: these were the error codes displayed during the last crash, I am not sure if they are always the same because I generally ignore languages I don't understand
After this I am able to successfully run what used to be called scandisk 100years ago (I forget what it is called now) and continue my gameplay.
This began in late May at an average of 2-3 times a month, but is now much more frequent (5 times this weekend alone).
I am running a Gateway PC with Vista 64-bit.
Thank you in advance for all your help,
