I have Kaspersky Internet Security. Do I really need this update from microsoft?

If I don't, is there anyway to permanently opt out of receiving this update?

No, you can skip this if you have Kaspersky working correctly.

The MSRT only scans for specific, widespread malware every month when there is an update. Any decent AV product offers much more protection than MSRT.

AFAIK, there is no way to permanently opt-out. You can opt-out for the latest version (like any Windows update), but you'll have to do it again next update.
