BleepingComputer.com: Toolbar and desktop icons don't load completely

I recently reformatted the C Drive on the laptop. I was uninstalling the bloatware on the laptop and installing some programs. After a couple of reboots, the laptop doesn't like to start-up functionally. When during start-up in which the toolbar and desktop icons are loading, sometimes the toolbar is blank with lines which you don't see any of the icons on the toolbar and only half of the desktop icons load with pictures and some don't have the pictures, like have the page with the rip in the middle icon.

Whenever the toolbar loads completely, I click the start button and click on all programs, the program list freezes and just stays there. Whenever clicking the desktop icons the window pops up but also freezes too while loading. Usually I can exit/close the programs (in the lower right) and it doesn't freeze.
I had a problem before with the laptop but that was fixed during the format. However, the multiple forced shutdowns may have cause irreparable damage to the laptop.

I formatted the computer again and tried not uninstalling any programs but only updating Windows. It worked but when I reboot the laptop only once, it starts to break apart. It seems that the rebooting somehow is going wrong. I've formatted the C drive about two times so far. Should I reformat it again?
Safe mode works but I'm not sure where to go from there.

I would think that a diagnostic of the hard drive...might be in order.

What exactly do you use to reinstall Windows?
