on my xp pro sp3 machine i latly seen the thread ntdll.dll!rtlallocateheap+0x18c in explorer.exe useing 45% to 50% of the cpu when launching a program
some programs also hace this thread but only explorer.exe 's thread acts up
i have run mbam and gmer and spybot and they all have found nothing
also some times the thread acts up like 15 seconds after a program starts

I don't play around in Task Manager much...but, from what I've seen...it's not unusual for process usage to spike when a program is opened.
You can use Process Explorer to further look at any processes you have concerns about.

that the program that i have been using to find the thread but i still dont know why it is acting up

am i over reacting or is this normal
it also spikes up when doing some rendering video in handbrake and music in fl studio
when i terminate the thread explorer just hangs

I cannot answer the first question...that's for you to decide.
As for me, I think that it's normal for Task Manager to reflect spiking....not sustained levels...spiking when a program opens, works hard such as during rendering, etc.
When I render video, my video-editor takes much, if not all, of the CPU cycles. I don't run other programs while rendering, I just go to my other desktop.
Explorer, System Idle, Iexplore, and a few other processes...should be reflected as running as long as the system has booted into XP.

yes but when rendering hanbrake gets 40% and explorer is using 60% specifally the thread ntdll.dll!rtlallocateheap+0x18c or known as ntdll.dll!RtlpWokerThread

using procmon i found that explore would be reading meta data on each file that the program opened but what does that mean
