Louis asked me, if I can come up with some new ideas here....
Run hard drive diagnostics: http://www.tacktech.com/display.cfm?ttid=287 (or http://www.bleepingcomputer.com/forums/ind...ive+diagnostic)
Make sure, you select tool, which is appropriate for the brand of your hard drive.
Depending on the program, it'll create bootable floppy, or bootable CD.
If downloaded file is of .iso type, use ImgBurn: http://www.imgburn.com/ to burn .iso file to a CD (select "Write image file to disc" option), and make the CD bootable.
NOTE. If your hard drive is made by Toshiba, unfortunately, you're out of luck, because Toshiba doesn't provide any diagnostic tool.

Couple of questions...
How long ago did this problem start?
Have you tried system restore?
Now...
Let's go back to chkdsk then.
From the error message, it looks like something is blocking the access to your drive.
Let's try something....
Go Start>Run (Start Search in Vista), type in:
msconfig
Click OK (hit Enter in Vista).
Click on Startup tab.
Click Disable all
IMPORTANT! In case of laptop, make sure, you do NOT disable any keyboard, or touchpad entries.
Click Services tab.
Put checkmark in Hide all Microsoft services
Click Disable all.
Click OK.
Restart computer in Normal Mode.
NOTE. If you use different firewall, than Windows firewall, turn Windows firewall on, just for this test, since your regular firewall won't be running.
If you use Windows firewall, you're fine.
Now, when you're back in normal mode...
1. Click Start, click Run, type chkdsk /f /r, and then click OK.
2. At the command prompt, type Y to let the disk scanner run when you restart the computer.
3. Restart the computer.
4. Chkdsk will run.
Let me know, what happened.

Download GMER: http://www.gmer.net/files.php, by clicking on Download EXE button.
Alternative downloads:

Double click on downloaded .exe file, select Rootkit tab and click the Scan button.
When scan is completed, click Save button, and save the results as gmer.log
Warning ! Please, do not select the "Show all" checkbox during the scan.
Post the log.
IMPORTANT! If for some reason GMER refuses to run, try again.
If it still fails, try to UN-check "Devices" in right pane.
If still no joy, try to run it from Safe Mode.

I did try sysyem restore, but it asked me to do chkdsk.
im not sure how bluetooth, MSN, rainmeter, and my antivirus could screw my computer but i disabled them anyway
disabled non-microsoft services.
Tried ChkDsk, but i think it refused to run cause the command prompt just flashed, so i proceeded to the next step first.
I restarted and chkdsk didn't run.
please ignore log.log
I ran GMER and got this application has stopped responding then it BSOD'ed.
Tried again, got the same error (but without BSOD thank goodness!)
tried to start again (BSOD! PAgefault in unpageded area or something)
tried again, without devices, its working, attached (gmer.log)
Real life is getting in the way, so i won't be able to respond till Wednesday evening.

GMER log looks good.
Please, undo all changes you made in "msconfig". They were done only for testing purposes.
Do you have Vista DVD? If you don't, can you borrow Vista DVD?

It prompted to C: \ would be a mistake and therefore could not perform a system restore, I wanted to start chkdsk (which obviously did not start well.) (Note System Restore does not occur).
