Is it safe? To install customized xp desktop themes

Provided the source of the theme is a trusted one, then yes.
Under Windows XP (which I'm assuming you're using) it is not possible to install customized themes that haven't been digitally signed by Microsoft without patching the uxtheme.dll file. Luckily, most themes available for download come with a utility to do this automatically.
Here are several that I've used before and can vouche for their safety:
Vista Inspirat: A Windows Vista inspired theme with extra goodies like a transparent taskbar.
CrystalXP: From the same people as above, but based on Mac OSX
WinCustomize: Full of interesting themes and other goodies. My favorite is Bootskin, which allows you to install custom Windows loading screens.
Currently, I use Royale Noir, a variant of the Royal Blue theme. These themes were created by Microsoft but never officially released.
